Dementia Matters Here supported by the National Lottery Community Fund is implementing a county-wide approach to community-based support.

We will deliver a sustainable step change in dementia support across the county and will act as a model which could be replicated in other regions.

We will be increasing the number of meeting centres, memory cafes, carers groups, increasing individual support and assessment for people affected by dementia, improving access to information, joining up a services collaboration, including access to on-going support, assessment and transport, campaigns to raise awarenes and prevention of dementia and to highlight the benefits of early diagnosis and developing additional dementia friendly communities.

We will be holding a number of events across Herefordshire, throughout the next 3 years, to enable people to have their voices heard and to bring organisations together to listen.
